Please enter my team in the Lake Winni Relay race.  We agree to assume all responsibility 
for all risk of damage or injury to us as participants in this event.  In consideration of acceptance I hereby 
for the team, myself, our heirs, executors and administrators, release and discharge any and all individuals
and organizations associated with this race, from all claims, damages, rights of action, present or future, 
whether the same be known, anticipated or unanticipated, resulting from or arising out of, or in incident to,
our participation in this event.  I hereby certify that we will not participate in the race unless we are 
physically fit and sufficiently trained for competition.  I understand that pacing, bicycles, skateboards,
baby joggers or strollers, roller skates or blades and animals are not allowed in the race and we will abide 
by this guideline.  Also we understand that there is no drinking of alcoholic beverages or public urination
allowed anywhere on the course during this event.
